In this episode of Toughest Place To Be…, Irish A&E nurse Berna Breen travels to Tegucigalpa, Honduras, to work in the emergency department of a public hospital. Berna comes face to face with the traumatic effects of a gang war that marks this city out as one of the most dangerous places on earth.

Filming took place over one week in Honduras, following Berna through her shifts in the hospital. It is here Berna also comes face to face with the victims of this bloody gang war. Away from the intensity of the frontline, Berna experiences everyday life by living with the family of a fellow nurse. Context for the violent situation in Tegucigalpa was provided by using news archive.
